Please give me ideas to make a new playthrough of GTA but harder, something chellenging

You should try out the GTA IV Minimal Kills Challenge. Seems like quite a challenge

Choose to kill Playboy X. Become good friends with Dwayne and call up for his back up when doing the side missions, like Brucie's car thefts, Little Jacob's drug package runs or Police Vigilant missions. You can even use them to have shoot-outs with other gangs in the street.

Only use the knife to fend off enemies.

But seriously, toggle the HUD off, reject the pick-up of first-aid kits and armor vests and basically all of the ones previously mentioned are all very good. Another good one is to not use any vehicles to get to destinations, but sprint to all of them. This can't possibly work for ever single scenario, but when possible, try to do this. It may be tedious, but completing the entire game like this is quite rewarding and satisfying, IMO.

Well, I did that in a previous walkthrough, was fun actually, but I need more challenge

Try to drive a car without any damage.
Alternative, If you want some fun like Police RIOT ok then fight with pistols.

I tried a play-through where I didn't steal any cars (other than mission mandated ones or ones given to me). I managed to unlock Algonquin before giving up...not stealing vehicles is pretty much against the spirit of the game lol.

Here are some things which will make the game harder.

Use only a Glock pistol or a Combat pistol only
Turn off HUD
Do not damage your car while driving on the streets
Do not use cheats
Do not buy weapons just pick weapons from free pick ups

When a police officer stops you. You must hand yourself in.
You cannot use cover.
You must keep all available friendships above 80%, as a side product of this you must accept every request for activities.
You must abide by traffic laws.

Turn HUD+Radar off. Keep GPS voice ON.

- Obey all traffic laws.
- If you have auto-aim/lock on, turn it off.
- Use only a pistol, no other weapons (I've actually done this before, very fun in large shoot-out missions, although when it comes to shooting down a helicopter, it can be awfully tricky.)
