You can reinstall the OS and play most games with no problems. The only thing that can and will fudge up is the third party software and possibly the game updates as some patch installers will not be able to find the directory in which the game has been installed.
AFAIK for GTAIV only patches 6 and 7 require the game path to be found in the registry or otherwise they'll just abort the process. Of curse, you can easily unpack the installer and just apply the new files over the olds one manually. Again, AFAIK none of the patches add anything of significance to the registry, anyway.
If you have game version 1.0.6. or 1.0.7 you don't need the separate RGSC app, only reinstall GFWL. If you don't need on-line, just use xliveless. Of course, Securom will ask for activation once you reinstall the OS, so the proper procedure is to a) revoke your current Securom license b) realize you may be losing on of the GFWL activations which may and may not be limited to 5 or 10 (don't shoot the messenger) c) simply run the launcher once in windows 7 and input the product key to have it activated via Securom d) once you install GFWL, it may prompt you to input the product key one more time. If you don't get prompted, then most probably you haven't lost your previous GFWL activation
