From the games I've played till now, Dead Space 2 was the most gory. I'm still looking forward to Manhunt.

Gears of War is pretty bloody but it's not disturbing like Manhunt.

Old game, English football (soccer) hooligan game. Strategy game, really quite hard, top down 2.5D view. Levels all around Europe. Only on PC though.

You basically loot shops, rob houses, take over pubs, vandalize streets, fight opposing fans, fight the riot police who have tear gas, buy drugs, get drunk, rally other fans / hardcore hooligans, go to brothels, weapon shops, where you can buy anything from knives, chains, bats, molotovs, guns blah blah blah. You have the ability to control separate factions of your group. It's really quite fun and very violent.

EDIT : Oh yeah, and you can drive cars. AI isn't great though, you do have to stay on top of the orders you give the hooligans. Telling them to run round obstacles before attacking the target sometimes is needed if your group is large. But it also has online play capabilities, which i have never got to work . The games pretty rare and unknown. i have read in online forums people talking about finding online games but i have never managed it. Not even 100% sure how to work the lobby.

Rockstar should clearly make the new version of Hooligans.

Soldier of Fortune 2 is probably the most graphic in it's actual gore.

For me it's Manhunt 1 and Thrill Kill. The later one was a game that never had an officiall release and you fight different kind of weird-looking psychopats. There's much blood and you execute fighters at the end.

Harvester was pretty disturbing just like Phantasmogaria, those games with real actors are always amazing.

I forgot about Carmageddon myself D:

Especially Carmageddon 1 had some nice gore with intestines flying out and THREE EYEBALLS a person.

They also had a plan of being able to drive into pregnant women causing the fetus to stick to your windscreen, but yeah they cancelled it. Lame.
