For me, the 3 most hated/hardest missions were Pegorinos Pride, The Snow Storm, and Flatline
Pegorinos pride is annoying because its a long ass mission. You have to drive (not take a taxi, drive) to the location. You then had to snipe all the enemies, who moved everywhere. Then, you had to save Pegorinos ass. And finally chase after the car and get back whatever you needed to get back (i forgot now) The first time i did it, i did it with ease, but when i chased after the cra i read wrong i shot the car until it was on fire, destroying whatever was in the car. The next few times i just kept dying because there was no cover after you ran out of the building you sniped from, making me lose half my health.
The snow storm was easy when you had to kill all the enemies and collect the coke. It was when the N.O.O.S.E came and stopped you from escaping. they all parked outside and i just couldn't get to a car. I looked it up and found out there was a god damn boat parked outside. I put on armor and health and just made a run for the boat. after that it was easy
Flatline was pissing me off because when you kill Anthony, it says "walk out calmly without suspicion" or something along those lines, but whenever i did, the guards started shooting you. I tried walking out different ways but no matter what the guards start shooting you. Its like the game tricks you. You also have to get out quickly or more cops will keep on coming in.
Other then that, i quite like gtas missions. There not too easy, nor not too hard, just fun. Not saying the other gta games aren't like that

Most of the missions were not that hard... It's just A) When you play the game for the first time it's hard figuring out WHAT you should be doing while people are trying to kill you, and B) GTA IV didn't have TBOGT's check point system yet, so you had to start the mission over from scratch if you failed. This was more frustrating than some of the missions themselves because you literally had to go all the way back to where the mission starts before you even attempt it again.
Also, some of the wonky physics work against you when they shouldn't.
Like others said, you will fail certain missions simply because you don't take a tight corner the right way causing the car to flip over letting your target escape, or just "glitching" the mission (you can't progress because the scripting was never designed for the player to get stuck in that way, etc).
For example, in the mission where you kidnap Gracie... She flipped the car onto a guard rail and that was it. I couldn't get the car free, so I had to start over...
