First of all, this is impossible without the Windows Live Wrapper, which you can find here http://blog.gib.me/ (big thanks to Gibbed aka Rickz for that) Open GTA 4 with the wrapper and open cheat engine (http://www.cheatengine.org/) 1.While in-game with full health, open the Cheat Engine window 2.Click the flashing icon and select the GTA 4 process in the list. 3.In the Value bar fill in 200 4.Click the Value Type dropdown menu and choose Float 5.Click first scan (this might take a few seconds) There should be a list of values at the left 7.Go back to the game and lower your health (jump in front of car etc.) 8.Go back to cheat engine 9.Click the Scantype dropdown menu and choose Decreased Value 10. Repeat steps 7-9 untill you have only a few values in the list. 11. Select all the values that represent your health (they should be betwoon 0 and 200, for example 189 after a small amount of damage) 12. click the red arrow 13. check all the "Frozen" checkboxes 14. Double click Value and change it to something high like 5000 15. Go back in-game and enjoy not dieing Note: Does not work for falling off high buildings because that just ignores how much health you have. You are protected from bullets, explosions,melee and any other physical damage. I would love to make this into an easy trainer, and Cheat Engine can do this but it doesn't seem to work properly so far. Example: http://www.youtube.com/watch?v=AKBYFpF72W8

It won't affect multiplayer, as the author stated on his website, he specifically disabled use of his wrapper in multiplayer, and rightly so.
I don't get the cries for a trainer either, as this is very easy to achieve.
Follow the steps mentioned in the OP, change your health value to 5000 or even better 50.0000, save your game, and the next time someone shoots at you, your health is good for a long, long time, (because obviously it's not going to be locked again).
