Answers

1. This was done during the mission Triads and Tribulations. That Leone guy got in the Kuruma
2. Since I drove the Kuruma to that side of the map. He obviously had to come there. Because he was in my car.
3. He stepped on a land mine.

This happened when I was doing the mission "Pick up" just when the cutscene ended i heard this woman screaming when I looked she fell "thump" into the Kuruma. I replayed and took the screenie.
