Katie, Michelle, Barbara, and Helena will get a progress boost to 45% if they are at or below 0% when CJ collects the 50th oyster. These 4 girls can also be re-met if their progress drops too low (I'm not too sure on the details, but killing them probably prevents their return). Denise and Millie are gone forever under normal circumstances if ignored for too long. Rescuing Denise will be pretty difficult to do without fiddling with the standard game elements - cheating, trainers, editing the save, or running a custom code, that sort of thing. The easiest fix would probably be San Andreas Control Center - a Trainer. Use it to set Denise's progress to something above 0 before she dumps CJ and I suspect she'll stick around.
The more drastic fix is to edit the bits in the save file that specify which GFs are available. I'm not aware of any PC fixes for dead/killed/never available girlfriends other than manually editing the save file. We just recently tested the theory on PS2 using cheat codes. The test was to kill Denise and bring her back. The procedure that worked was to repair her progress global and stat (the slider in SACC), and throw the flag that puts her back on the map. It worked perfectly as long as we wrote the whole dword on the global instead of just the last byte (which is the only reason the old PS2 cheat didn't fix the killed GFs).

Strange. I would have thought bumping her stat would have helped. I wonder what the trainer changed. But then again, I was testing an already dead GF, not an impending dump. I can understand your reluctance to use a trainer on your good save, they often introduce their own set of glitches, and most of the time it's a matter of pride. If you are interested I can probably fix Denise with some precise edits of a save file, which might work better after she's dumped him. Otherwise, yeah, reverting to a previous save is probably the only option.
I'm afraid the advice on how to loose muscle was counter productive, but it won't matter all that much since fat is easy to get rid of. Go to the gym and ride the exercise bike until all fat is gone. You'll probably need to exit and return to the gym every 5000 units so CJ doesn't get kicked out for too much exercise in 1 day. When all fat is gone get in a car and park someplace far away from any trouble. Leave CJ sitting there and go get something to eat or do some chores or something. Check on CJ in about 20 to 30 minutes and there should be an occasional stomach growl followed by a hit to CJ's muscle. If CJ has fat then it will burn before muscle, so exercise it all away first.
If you'd like to be more productive during this time then I suggest an oyster collection; this usually burns a lot of energy. The Paramedic mission in Montgomery is just about the same time required to start getting into a hunger issue unless you are very efficient at shortcuts to Dillimore. This is a good opportunity to go for a large In-a-Row bonus for the Taxi mission - lots of cash if you keep the streak going. Trucking missions are good for burning fat but they are also a bit risky. You don't want to save, eat, snack, date, or get busted or wasted while burning off extra fat or muscle.
As far as Helena is concerned, there are FAQs and Guides available that specify which body type she prefers; I forget her preferences since I usually have the GF bonus fairly early - before CJ meets Denise.

I exercised 3 sessions till limits having 0% fat and hardly 4% muscle decreased. I was running on foot and swimming between the gym sessions in a hope that muscle would start decreasing rapidly but nothing much happened. I'm not very sure if I was starving CJ. I ll drop this savegame as soon as I beat the mission End of the Line. I'm planing to start a new game for girlfriend experience without any conditions. For this save game I have these conditions to make the game more exciting 1. No Busted 2. No Wasted 3. No Mission Failed 4. No Frequent Saves 5. No Useless Saves (Without certain amount of progress) *. And of course No cheats, trainers and bla bla bla... I cant enjoy girlfriends with this save game. Mainly Katie n Barbara(Keeping Weapons after getting Wasted/Busted was useless for me) and even for the Millie I used Gimp suit trick so not much of real dating been done.
