If you headshot NPC in GTA IV, he/she dead immediately.

But, I have one question. Have you (or Niko Belic) been headshoted by peds or police to make him dead instantly?

No never happened to me, think I would have failed a lot of missions if this was the case.

I don't think it is even possible to have a protag in any GTA game die instantly from a headshot from an enemy. If that were the case then the games would become increasingly difficult.

Would be nice to have a purist mode option where a headshot would mean instant death, and have a limited time after missions to visit hospital to have other wounds treated. We can just assume that adrenaline gets you through the mission with bullets in the arms, body, etc.

OPTION is the key word here, of course.
