I hate paramedic missions in gta 3.
I just reached level 5 when my ambulance is overturned and then exploded.

Is there a way for example modifying the savegame to complete the mission?

Yes, there is a way to get past the mission without doing it manually. First, download TimeVault - you'll download a file called mainTIMEVAULT.scm. Now, go to the GTA III folder, click the data folder, and find a file called main.scm. Rename it to (1)main.scm. Now, put the mainTIMEVAULT.scm file in the data folder, and rename it to main.scm. Now go and start the game, get in the Ambulance, start the Paramedic missions, and you should have passed them as you click the start mission. Now go and save the game, exit it, remove the current main.scm (only if you wish to remove it, in case there will be more missions you can't pass, better rename it TimeVault.scm) and rename the original (1)main.scm back to main.scm and carry on with the game!

Off-topic, since you are having a PC version, I present an alternative that is more "fair", at least as I consider it to be.

Upload your save file on any file hosting service (preferable Forums service - Samutz's Save'N'Play) and post a request for a mission to be accomplished in this topic, and then wait for someone to do it. After that just download the file with the accomplished mission back to your PC.

Keep this note in the future, should you require any help. All III era game sections (except for LCS, but that one's in VCS), as well as IV era game sections, have the Save'N'Play section.
